Yoğun Bakım Ünitesinde Deliryum ve Ortalama Trombosit Hacmi (MPV) İlişkisi: Güncel Bulgular ve Klinik Önemi

Özet

Yoğun Bakım Ünitelerinde (YBÜ) yatmakta olan hastalarda sıklıkla görülen geçici nörodejeneratif bir bozukluk olan deliryum, yüksek morbidite ve mortalite oranları ile ilişkilendirilen kritik bir durumdur. Deliryum tablosunu erken teşhis etmek ve yönetmek, hasta prognozunu iyileştirmek için oldukça önemli ve gereklidir. Ortalama Trombosit Hacmi (Mean Platelet Volume-MPV), trombosit aktivasyonu ve inflamasyonun bir göstergesi olarak kabul edilmektedir. Bu gözden geçirme çalışmasında çeşitli veri tabanlarında yer alan Ortalama Trombosit Hacmi, deliryum ve yoğun bakım üniteleri ile ilgili araştırmalar taranmış ve MPV ile deliryum arasındaki ilişkiyi inceleyen ve 2024'e kadar yapılmış güncel çalışmalar Scopus, Taylor & Francis, Science Direct, TÜBİTAK ULAKBİM, PubMed veri tabanları aracılığıyla derlenmiştir. İncelenen çalışmalarda, MPV'nin yüksek değerlerinin deliryum gelişimi ile ilişkili olduğu ve yoğun bakım ünitelerindeki hastaların prognozunun etkileyebileceği yönünde bulgular elde edilmiştir. Sonuç olarak, mevcut literatür MPV'nin yoğun bakım ünitelerindeki hastalarında deliryumun prognostik bir belirteci olarak potansiyel değer taşıdığını öne sürmektedir. Ancak, bu ilişkinin kesin olarak belirlenmesi ve klinik uygulamalarda standart bir ölçüt olarak kabul edilmesi için daha geniş kapsamlı ve prospektif çalışmalara ihtiyaç duyulmaktadır.
Delirium, a transient neurodegenerative disorder frequently observed in patients hospitalized in Intensive Care Units (ICUs), is a critical condition associated with high morbidity and mortality rates. Early diagnosis and management of delirium are very important and necessary to improve patient prognosis. Mean Platelet Volume (MPV) is considered an indicator of platelet activation and inflammation. In this review, studies on Mean Platelet Volume, delirium, and intensive care units in various databases were examined, and current research investigating the relationship between MPV and delirium up to 2024 was compiled from Scopus, Taylor & Francis, Science Direct, TÜBİTAK ULAKBİM, and PubMed databases. The reviewed studies found that high MPV values were associated with the development of delirium and may affect the prognosis of patients in intensive care units. In conclusion, the current literature suggests that MPV has potential value as a prognostic marker of delirium in patients in intensive care units. However, more comprehensive and prospective studies are needed to definitively determine this relationship and to establish it as a standard measure in clinical practice.
